The application
What does an adoption application look like? There is a sample on page 191. Your application will probably look different, but this will at least give you some idea of what you can expect. Some of these questions will be repeated or expanded upon during your home study.
- Name
- Other names you may have gone by (maiden name, or other married names included)
- Address
- Birth date
- Social Security number
- All employers for the last 5 years
- All addresses for the last 5 years
- Drivers license number
- Dates and places of any marriages and divorces
- Who are your family members, and where do they live (immediate family only)
- Current insurance policies, and limits on your policies (this includes auto, home, life and health), along with policy numbers
- Any assets held, including real estate, stocks, bonds, CDs, checking, savings, etc.
- Any pets in the house, and are vaccinations current? You will not be allowed to take placement of a child unless all vaccinations are up to date.
